Patient Safety Incident Response Framework Benchmarking

The Patient Safety Incident Response Framework (PSIRF) was published in August 2022, alongside a preparation guide that articulates a phased approach to transition. PSIRF fundamentally shifts how the NHS responds to patient safety incidents for learning and improving.

This report examines the implementation of PSIRF across 59 NHS organisations from across the TIAN client base (4 ICBs and 55 provider trusts).
